From the Office of the University Registrar, go to Student Forms to access the following:

  • Address Correction Form

  • Official Transcript Request

  • Alternate Spring Commencement Application

  • Re-enrollment Request

  • Leave of Absence Request

  • Registration Drop/Add Form

  • Name Change Request

  • Undergraduate Nondegree Application
